Good afternoon! This is the last of the Men's Health email series. You can find all of them on the website.
Consulting with me at IWHO starts as a conversation. You do not need to arrive knowing whether the issue is testosterone, sleep, weight, stress, medication, or something else. Start with what you have noticed and what you want help with.
At a confidential consultation, we can clarify the concern that brought you in. Is it low desire, difficulty getting or maintaining an erection, fewer morning erections, fatigue, changes in strength, weight gain, poor sleep, or a combination? These concerns can overlap, but they are not interchangeable, and the distinction can help guide what needs to be evaluated.
We can review relevant health history, medications, sleep and weight concerns, prior testing, and your goals. If testosterone deficiency is being considered, symptoms alone or a single low result are not enough to establish the diagnosis. Clinical guidelines recommend confirming low testosterone with repeat morning testing and interpreting results in the appropriate clinical context.
It is also important to be clear about what TRT may and may not do. In the TRAVERSE Sexual Function Study, testosterone improved sexual desire and activity in men with confirmed low testosterone and low libido, but did not improve erectile function compared with placebo. That is one reason an erection concern should not automatically be treated as a testosterone problem.
The next step depends on what we learn together. It may be further evaluation, reviewing existing results, discussing treatment options, or considering other possible contributors. TRT is not assumed, and no single treatment is right for every man.
I link research articles because I want you to be able to read the science behind my clinical thought process. I use research and data, and I also want to hear your experience, concerns, and goals. Those are both important parts of a thoughtful evaluation.
If you have been putting off the conversation because it feels personal or uncomfortable, you are welcome to bring it up here. Sexual health, energy, sleep, weight, and hormone concerns are appropriate topics to discuss with a clinician.
